Insurance
I'm an out-of-network provider. If you have PPO insurance that allows you to see an out-of-network provider, I would be happy to provide you with a superbill for your reimbursement. Please contact your insurance company to inquire about your reimbursement rate.
Payment
I accept all major credit cards, HSA, and Zelle as forms of payment.
Cancellation Policy
Please cancel your appointment at least 24 hours in advance to avoid a late cancellation fee.
